NHM Student Employee Confidentiality Agreement

This form should be signed by any student workers in NHM before gaining access to confidential records in their duties as student employees or volunteers in the Department. The NHM fulltime faculty or staff member who is the student’s direct supervisor is responsible for obtaining and keeping a signed copy. The agreement remains valid through June 30th of each academic year and can be renewed annually should the student’s duty require continued access.

NHM Student Employment Requests

This form is to be used by NHM faculty or staff members, who intend to hire a graduate or undergraduate research/teaching/administrative assistant. For timely payroll processing, it is recommended that this form be submitted to the NHM administrative coordinator at least 60 days prior to the first day of employment.

NHM Lenoir Hall Guest Access and Utilization Request

This form is used by an NHM fulltime employee to request for authorization of a non-NHM person’s access to and/or utilization of the physical resources of Lenoir Hall under the NHM employee’s supervision. Any access or use without a signed form in file with the NHM administrative coordinator will not be allowed. One request form can be used to grant access for up to two persons involved within the same scope of work. Separate forms should be submitted for additional persons for the same scope of work, the same person for a different scope of work, access to multiple rooms, or periods ranging multiple semesters.

NHM Special Topics Course (NHM 691/791) Approval Form

All special topics offerings must be submitted to the Chair of the Department and approved by the Graduate Faculty before being placed in the schedule for the first time. A course may not be offered more than three times as a special topics course. Offerings beyond the third time must be submitted for approval as a regular course through graduate C&P.

NHM Evaluation Rubric for Peer Evaluation of Teaching

As part of the institutional assessment for SACS, this rubric was developed to assist the evaluation of NHM graduate students’ ability to teach. The faculty mentor of an instructional graduate assistant is responsible to conduct a classroom observation once during the semester, and return the form to the GPC.

All instructors of NHM courses should use this checklist to ensure their syllabi are in compliance with the SAS syllabus policy.

SAS Student of the Month Nominations

NHM faculty and staff can use this form to recognize an undergraduate or graduate student who demonstrates exceptional performance in educational, research, and service activities that exceed the norm. Selection and notification will be coordinated by the SAS Associate Dean. It is advised that you do not inform the student about the nomination.
